Introduction

Crescent Dental Ltd aims to provide care of a high standard to all patients. This will be accomplished by working with patients to create a safe and friendly environment. The responsibility of ensuring this statement of purpose lies with Dr Jennifer Berndt.

Aims and Objectives:

  1. To provide high quality dental care; including consultations, x-rays, treatment of periodontal disease, fillings, prosthetics (e.g. crowns, bridges, dentures) and root canal treatment (endodontics)
  2. To focus on prevention of dental disease by promoting good oral health to all patients attending our practice for care and advice.
  3. To provide patients with an experience and environment that is comfortable, friendly and relaxing.
  4. To understand and meet the needs of our patients, involve them in decisions about their care and encourage them to participate fully.
  5. To involve other professionals in the care of our patients where this is in the patient’s best interests; for example, referral for specialist care and advice.
  6. To ensure that all members of our team have the right skills and training to carry out their duties competently.

Diagnostic Services: In all cases a dentist will assess and carry out appropriate testing to reach a diagnosis for each patient. Diagnostic procedures include assessment of periodontal health, taking radiographs where necessary, soft tissue screening, and consideration of medical and social factors. All options for treatment will be discussed in detail with the patient and a treatment plan agreed and itemised for the patient to refer to.

Treatment of Disease, Disorder or Injury Services: Treatment will be carried out by a dentist, hygienist or other registered dental professional as consented and agreed on the treatment plan. Treatment is recorded and detailed accurately within the clinical notes for that patient. If the patient wishes to withdraw from treatment at any point then they are free to do so.

Treatments include: Examinations, Scale & Polish, Periodontal treatment, Fillings, Extractions, Root Canal Treatment, Crowns, Bridges, Dentures and emergency treatment as required.

Urgent Treatment/Out of Hours: The practice deals with all dental emergencies within 48 hours of any patient calling, however should a patient need urgent care when the practice is closed they should contact NHS Direct on 111 or Practice Plan out of hours on 0808 169 8117. When the practice is closed the answer machine message will leave instructions for patients for emergency care. The details for emergency care are also displayed externally on the practice should a patient attend when the practice is closed.

Cross infection control and disinfection: This is carried out in accordance with the latest guidelines and regulations to the highest possible standards.

Patients Confidentiality and Dignity

All patients have the right to privacy and dignity as a practice we actively seek to maintain patient confidentiality in line with GDC standards.

If patients do not wish to discuss aspects of their care or circumstances with receptionist on the main reception, staff will arrange to discuss these away from other staff or patients.

If a patient would like to speak privately about their care, they can arrange to speak with the practice manager on a suitable day/time, or another appropriate member of staff.

These activities will be carried out exclusively at the following location, using the surgeries and equipment provided and maintained by the Service Provider.

It is our endeavour as a practice to provide care at a time which is convenient to our patients and this is reflected within the opening hours.

Monday 8 am – 5.00 pm
Tuesday 8 am – 5.00 pm
Wednesday 8 am – 5.00 pm
Thursday 8 am – 5.00 pm
Friday 8 am – 4.30 pm

Patient Responsibilities

As a practice we create a safe and non-discriminatory environment where patients can attend. We do ask of our patients to treat all staff with the same regard that they are treated.

As a practice we will not tolerate any form of verbal or physical abuse, this could be in the form of shouting, profanities intimidation or other behaviour that results in staff feeling unsafe. Any patient found to disregard the rights of our staff to be treated with dignity and respect will be removed from the practice register. A patient has the right to appeal and this should be made in writing to the Practice Manager within 7 days.

Seeking Patient Views

As a practice we strive to improve our service for patients. Every year we undertake a patient survey and discuss the outcomes with staff, developing new methods of working based on patient comments. If patients wish to comment when the patient survey is not in process they are encouraged to fill out a patient questionnaire available from the main reception.

Complaints

Patients have the right to complain about a service they have received, all complaints should be treated with respect. Any complaint made by a patient will be dealt with in a non- prejudice way, and fully investigated. The practice aims to deal with all complaints received within 30 working days, however on occasion a full investigation can take a longer period of time, if this is the case we will inform patients of any delay to their response. A request can be made to the Practice Manager if a patient is unsatisfied with the service for details on how to further complain, all contact details are displayed in patient waiting areas.

Staff details

All dentists are suitably qualified for the position they hold. Dental Nurses are required to attend college through Learn Kit Ltd until qualification. Both dentists and Nurses are required to maintain their continued professional development in line with G.D.C requirements.

Organisational Structure

The Practice Management team, ensures that the GDC registrations of all dental care professionals are current, that their continuing professional development is current and that all the dentists’ individual indemnity insurance is current with the adequate coverage.

All dentists at this clinic are self-employed associate dentists registered with the GDC, covered by individual indemnity insurance and/or defence organisation membership.

Crescent Dental Ltd is the registered provider of the service; Crescent Dental Ltd does not enter into any form of contractual agreement or arrangement with a patient. Contractual agreements/arrangements for the provision of dental treatment (in the form of a signed treatment plan) are solely between the patient and the dentist. The dentist is responsible for the treatment and advice provided.

No treatment plan guarantee is offered by Crescent Dental Ltd. Treatment plan guarantees are offered by each dentist on a case by case basis based on that dentist’s assessment, and are subject to the patient complying strictly with their dentist’s advice and guidance.

Crescent Dental Ltd would like to reassure patients that their best interests are our priority. Crescent Dental Ltd have controls in place in the event that an individual dentist is unable to continue treating patients (e.g., through illness).

Terms and Conditions for the provision of dental treatment

Dentists reserve the right to refuse to treat a patient if they consider that the patient has insufficient understanding of the treatment, or they consider that a patient’s expectations are unrealistic and not clinically achievable.

The GDC publication Principles of Dental Team Working Page 6 Clause 3.1 states

All members of the dental team who have to register with the GDC are individually responsible and accountable for their own actions and for the treatment or processes which they carry out.”, and also Clause 3.2 states

As a registered dental professional you are also responsible for the actions of any member of the team you lead or manage who delivers care to the patient but who does not have to register with us (for example receptionists and practice managers).
